Exhausted is a status effect you suffer from after any of the following:

There is no way to prevent this status effect from hitting you after you've completed any of the above actions. However, the exhausted status' only effect on you is to prevent you from using any of the above abilities again until you've recovered. Note that this one status effect prevents all three options: having recently berserked will prevent you from calling upon Ru's abilities or casting Death's Door to survive in your weakened state.

History
