Primal Wave is a monsters-only spell that causes the tides of the Shoals to surge into you, dealing significant damage, turning the terrain beneath you to shallow water (if it isn't already), and pushing you around. Fortunately, it cannot knock you into deep water, and it deals 40% less damage if your character is either aquatic (merfolk, octopode) or does not breathe (mummy, gray draconian).
