The spear trap is a trap which fires +0, +0 spears when stepped upon by a player or monster. They hold 2 - 7 spears and deal 1 - 26 damage to you (monsters only take 1 - 4). They can be disarmed with the Traps & Doors skill.
Repel Missiles and Deflect Missiles both reduce the chance of this trap being able to hit the player. Flying characters never trigger them.
Tips & Tricks
If your HP is around 26 or lower (especially if you are lightly armored) and you are in no immediate danger, consider waiting until your HP returns to about 30 or so before continuing your explorations or even moving to a more defensible position; when you're low on health, all it takes is a single spear trap to kill off a promising character.
